Program Eligibility
The patient must meet the following criteria to qualify:
  • Be a legal resident of the U.S.
  • Has no health insurance or no prescription coverage.
  • Not eligible for Medicaid, ADAP, or other drug assistance programs.
  • Income must be lower than $18,000 (1-person household), $24,000 (2-person household), $35,000 (4-person household)
Enrollment Process
Doctors or patients can call to enroll. An application is sent to the doctor's office for completion. You can also download the form from their website (see above link).
Re-Application
The doctor can call for up to 90-days supply of the drug at a time. After one year the patient must re-enroll in to the program again.
Comments
Once the application if received, it takes 3-4 weeks for the drug to be available. It can be shipped to either the doctor's office or the patient's home.
